The facility will submit a claim to your insurance for their fee. Other bills will be submitted by an anesthesia provider and possibly a laboratory/pathology provider. Please contact the facility for more information about other provider details so you can verify their participation status with your insurance and you may ask your insurance representative how much your out-of-pocket costs may be.
