Phone calls required for prior authorization of medications or medical services consume much additional time and effort beyond your office appointment. We may request that you return for an additional office appointment to initiate the authorization process or to appeal an unfavorable insurance or disability decision.
Completion of forms for disability, medical leave, life insurance, or any other form requiring Dr. Plumeri's attention or signature must be accompanied by a $25 prepayment.
